Job Summary

A Cardiac Surgery/Structural Heart Clinical Nurse Navigator works closely with the Cardiac Surgery Clinic team and Heart & Vascular Institute Executive Leadership team to build comprehensive cardiac services across UMCRH. This role provides expert resource, services management, cardiac patient care, and direct support to the ambulatory clinical team to facilitate delivery of cardiac patient care from referral to post-operative plan of care.

Primary Responsibilities

  • Provides education, information, and resources about the cardiovascular diagnosis and the treatment options at the initial consult and throughout the continuum of care.  
  • Provides support during consultation with surgical cardiologist and provides support during unexpected hospitalization, complications, or setbacks.
  • Triages clinical patient telephone calls and performs preop and postop calls.
  • Provides caring, individual attention, compassion, and empathy to the patient and their family.
  • Assesses new cardiovascular patient referrals to identify clinical needs, appropriateness, issues, financial resources, and care goals.
  • Obtains appropriate records and test results for proper patient evaluation.
  • Responsible for various aspects of coordinating patient care pre and post-surgery.
  • Provides ongoing assessment of patient care management needs through frequent communication with cardiac providers and keeps others on the care team updated on patient status and treatment concerns.
  • Prioritizes care management activities in order of greatest patient need and system need to achieve optimum quality and cost outcomes.
  • Assists with problem solving and addressing any issues in daily workflows that may impede timely patient care or turnaround.
  • Assures new patient standardized orders are updated and monitors for deviations.
  • Develops and implements plans for monitoring referrals and tracking status through preop process to time of surgery.
  • Responsible for analyzing and tracking program specific data and key quality metrics and measuring data and quality metrics.
  • Develops and updates cardiac surgery/structural heart patient education materials.
  • Supports the needs of the UMCAP community in relation to cardiac education, screening and prevention.
  • Serves as clinical resource to assist in development of outreach strategies and community education.
  • Works with admin/clinical team to facilitate timely completion of any patient paperwork, letters, etc.
  • Performs all other related duties as assigned.

Qualifications

Education & Experience - Required

  • Bachelor of Science in Nursing (BSN).
  • 2-3 years of experience in Cardiac Surgery or inpatient/outpatient Cardiology.
  • Current Maryland Registered Nurse License, in good standing as a Registered Nurse (RN).
  • Basic Life Support- Health Care Provider (BLS-HCP).

 

Education & Experience - Preferred

  • 3 or more years of prior Cardiac Surgery or inpatient/outpatient Cardiology experience is preferred.

  • Self-Management skills.
  • Knowledge of Microsoft Office Suite Skills: Word, Excel.  
  • Working knowledge of ICD-10 (Medical Coding).
  • Ability to utilize standard office equipment.
  • Effective oral/written communication skills.
