Position Summary

Assist, counsel and prepare individuals with diagnosed disabilities, behavioral health and other barriers, for employment by providing them with self-assessments in the areas of skills and interests, and guiding them towards appropriate employment opportunities. Assist clients in preparing for work by reducing and addressing barriers to employment and by providing job readiness training and job retention services. Responsible for developing employment opportunities and connecting clients to job placements.

Essential Functions

  • Conduct and document minimum of weekly updates on client status, including enrollments, job development, job placements and retentions.
  • Provide counseling and support to enrolled participants with physical, mental, emotional or social disabilities and correspond with them weekly either face to face, via email, zoom, or by phone for updates and to track progress. External visits may be required.
  • Conduct outreach and maintain regular communication with ACCES-VR district offices and  counselors, HCBS and HARP providers, in order to build or sustain relationships with referral sources.
  • Collect appropriate documentation from HCBS providers, update Credible, and bill for services in database system.
  • Meet program targets and performance milestones established by management and contract in all case coordination areas of job function. 
  • Monitor attendance and update case-note activities and counseling sessions for assigned caseload in online database.
  • Assist with training and employment retention activities to support job development activities.
  • Evaluate clients’ abilities, interests, experience, skills, health and education to ensure they are job-ready. Provide ongoing vocational case management and retention services for an assigned caseload.
  • Host weekly job readiness workshops for clients, including interview skills, job search and other career readiness topics. 
  • Maintain knowledge of the needs and abilities of people with disabilities, as well as the laws and resources that impact people with disabilities.
  • Prepare written monthly reports for ACCES-VR counselors on status and updates of clients.
  • Schedule monthly presentations with referral partners in order to ensure that clients are being referred regularly.
  • Performs other duties as assigned.

 

Knowledge, Education & Experience

Formal Education: Bachelor's Degree

Experience: 1-2 Years

Criteria that would be desirable but not required:

  • Knowledge of workforce development, career assessment, job preparation.
  • Experience working with individuals with barriers to employment, including behavioral health, substance use, disabilities, limited work experience, and criminal backgrounds.

Salary $56,000
