Position Summary:

The Home Health Clinical Liaison serves as a consultant to healthcare providers and an advocate for patients and their families during the end-of-life process. The Liaison will provide education and support for Guthrie healthcare teams while facilitating the patient’s transition to inpatient and home-based hospice services. The Liaison will be the Subject Matter Expert for all levels of hospice care, including General Inpatient Hospice care.”

Experience:

  • Registered Nurse with experience in hospice, home health, or other home-based care setting preferred.
  • Minimum 2 years nursing experience required.
  • Ability to collaborate with healthcare teams, patients, and families required.
  • Ability to work independently required.
  • Demonstrated triage skills and basic nursing competencies required. 

Essential Functions:

  • Attend hospital rounding with a focus on end-of-life care to ensure timely referral to hospice. 
  • Work with case management teams to identify appropriate patients for referral to Guthrie Hospice services. 
  • Provide end-of-life and hospice program education to healthcare teams.
  • Participate in designated committee meetings as the hospice subject matter expert (SME).
  • Participate in Guthrie new staff orientation, presenting hospice best practices and contact information. 
  • Collaborate with hospital staff and physicians to identify patients appropriate for transfer to hospice care and facilitate timely transfer.
  • Collaborate with internal and external facilities, including hospital, SNFs and PCH, to support the various levels of hospice care, including respite, routine home care, and inpatient hospice (GIP). 
  • Serves as the SME for healthcare professionals, patients, and families regarding hospice admission criteria, ongoing service, and insurance coverage. 
  • Work with internal and external referral sources to ensure complete and accurate hospice referrals.
  • Provide care coordination for patients meeting hospice criteria, including GIP, routine home care, continuous care, and respite care. 
  • Participate in Guthrie sponsored research, performance improvement, or other quality programs approved by the Guthrie Hospice Director. 
  • Facilitate and support GIP hospice admissions in Guthrie Pennsylvania hospitals, including facilitating referrals, education of healthcare teams, care coordination during hospital stay, documentation review, family and healthcare team discussions, and any other GIP process within scope of a registered nurse. 
  • On-call coverage as per agency needs- identified by Hospice Director.

The pay rate for this position is $31.42 - $50.11 based on experience.
