POSITION SUMMARY:
As an Intake Coordinator at Innovive Health, you will oversee the inbound referral process from first contact to admission. Responsibilities range from collecting information from referral sources, processing documentation in a timely and effective manner, and collaborating with multiple departments to ensure our patients receive a smooth transition to home care. The right candidate will demonstrate excellent organization skills, is customer-focused with an ability to multitask in a high-volume, fast paced environment. You will work as part of a centralized Intake team and will be responsible for a primary assignment while providing cross-coverage across areas as needed.
What you'll do:
• Sort and process new patient home care referrals and resumptions of care for known patients received through various electronic portals, phone calls, faxes, and emails.
• Collects appropriate demographic information, checks insurance eligibility, and collects required documentation to ensure regulatory compliance with Medicare and Medicaid.
• Accurately data enters patient information in Electronic Medical Records (EMRs).
• Performs outbound calls to referral sources, providers, and patient facilities to ensure collection of required documentation.
• Collaborates with internal clinicians and other business partners to ensure timely adherence to patients care needs.
• Collaborate with healthcare providers, case managers, and referral sources to clarify service needs.
• Provide updates on referral status and address inquiries promptly.
• Accurately document all intake activities in the agency's system and maintain compliance with state and federal regulations.
• Ensure all required documentation, notes, paperwork, etc. are completed and uploaded to Salesforce and Healthwyse per processes.
• Works with multiple departments to ensure workflows are followed timely for authorization purposes per process.
• Assists with managing the workload across the department to ensure that referrals are handled in a timely manner. This includes providing back-up as well as coverage across different geographical assignments.
• Monitors referral portal regularly to ensure the timely review of new referrals.
• Adheres to HIPAA laws and maintains patient confidentiality at all times.
• Participates in appropriate continuing education courses, committees, and meetings as required.
• Serves as point of contact between clinical, intake, and vendors electronically and telephonically as required.
• Identify and address gaps or delays in the intake process.
• Continuously improve workflows to enhance efficiency and patient satisfaction.
• Assists with managing the workload to ensure that referrals are handled in a timely manner.
• Ensures issue resolution with referrals, vendors, and missing documentation.
• Monitors Salesforce queue and HealthWyse exports.
• Serves as the point of contact for all referral sources.
• Performs other duties as assigned.
What you need:
• Associate’s degree or equivalent in related work experience
• 1+ year experience in a healthcare administrative role
• Knowledge of medical terminology
• Proficiency in Windows environment, with emphasis on Word and Excel

Preferred Education and Experience:
• Basic knowledge of Medical Insurance – Medicaid and Medicare a plus.
• Previous experience in a medical office, or other medical administration role

• Occasionally required climb, balance, bend, stoop, kneel or crawl.
• Occasionally required to lift/push light weights (less than 25 pounds).

TRAVEL REQUIREMENTS:
• Ability to travel locally by car to office(s), events, meetings.
