Job Posting: Nursing Supervisor

Location: Warsaw and Rochester, Indiana
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (flexibility required; occasional evening/weekend hours and on-call responsibilities)
Salary Range: $69,700.18 – $86,344.96 annually

Position Summary:

We are seeking a compassionate and experienced Nursing Supervisor to oversee the delivery of medical care and wellness coordination for individuals in ICF/IDD group homes and community-based settings. This role ensures compliance with state mandates, supports direct care staff, and provides medical oversight and leadership for the health and well-being of those served.

Essential Functions:

  • Ensure compliance with all state requirements for Wellness Coordination and ICF/IDD group homes.
  • Oversee and monitor medical needs for individuals, including appointments and follow-up care (medical, dental, vision, psychiatric).
  • Lead efforts to enhance the quality and efficiency of care provided to individuals.
  • Support nursing staff in documentation and medical follow-up.
  • Advocate for individuals, families, and guardians as appropriate.
  • Collaborate with medical professionals, including the Medical Director, to address health needs.
  • Ensure accurate and timely documentation from internal and external staff.
  • Maintain compliance with agency policies and all relevant regulations.
  • Provide ongoing training for Direct Support Professionals (DSPs) on topics including medication administration, first aid, preventive healthcare, and more.
  • Teach Med Core A and B to DSPs.
  • Review and approve wellness documentation submitted by LPN Wellness Coordinators.

Supervisory Responsibilities:

  • Direct supervision of assigned employees.
  • Responsible for interviewing, hiring, performance evaluation, discipline, and termination decisions per agency policy.

Collaboration & Relationships:

  • Work closely with internal teams, referral and funding sources, and the broader medical community.
  • Serve as a resource for other departments regarding medical and wellness procedures.

Requirements:

  • Licensure: Current RN license in the state of Indiana.
  • Experience: Minimum 2 years as an RN; 1 year of supervisory experience preferred; experience with I/DD population preferred.
  • Skills:
    • Strong decision-making and leadership skills.
    • Effective communication in written and spoken English.
    • Comfortable with electronic documentation and basic computer applications (word processing, spreadsheets, email).
  • Physical Requirements:
    • Ability to lift up to 50 lbs as needed.
    • Use of assistive devices such as Hoyer lifts.
    • May be required to drive a modified van or other agency vehicles.
  • Other:
    • Valid driver's license, proof of insurance, and reliable transportation.
    • Willingness to work flexible/non-standard hours.
    • Availability for on-call responsibilities.
    • Must pass background check, drug screening, and reference verification.

Additional Responsibilities:

  • Attend required trainings and meetings.
  • Lead assigned committees or projects.
  • Track and report on departmental goals and outcomes.
  • Maintain professionalism and confidentiality at all times.
