Schedule: M-F, 8:00 am - 4:00 pm (evening/weekend hours as needed)

Pay: $18.03/hr ($37,500 annually)

Summary

The Family Support Coordinator is responsible for providing home-based and community-based family service coordination services. They work to cultivate relationships between our participants and their families to their local communities for engagement and services.

Essential Functions:

  • Develop individualized Family Service Plans with the family they are assigned to in a timely manner
  • Maintains regular monthly contact with the child participant/family and makes face-to-face visits quarterly (at a minimum)
  • Coordinates continuity of care and investigates alternative resources
  • Assists with obtaining transportation for appointments related to the child's service plan
  • Assists family in identifying needs, provides disability information, coordinates various services, and links family to service delivery systems
  • Enhances child/family's skills and abilities in procuring services to meet the child's needs
  • Promotes development of provider resources where services are incomplete or non-existent
  • Attends quarterly field supervision, monthly staff meetings, and weekly one-hour individual supervision meetings with the Program Coordinator or Director.
  • Completes at least 8 hours of agency orientation and 8 hours of target population training within five working days of employment. Completes 24 hours of FSC training and 8 hours specific to Early Steps
  • Completes and submits all IFSP forms on time (5 days after the meeting)

Education/Experience Requirements:

  • Bachelor's or Master's Degree in a Human Service related field.
  • Experience working with children with disabilities is preferable.
  • Must show empathy and understanding toward and protect the confidentiality of children/families.

Skills/Qualifications

  • Excellent time and priority management
  • Ability to collaborate with participants and their families as well as colleagues
  • Experience with Microsoft, including Teams, Outlook, SharePoint, etc.
  • Critical thinking and problem-solving evidenced by past experiences
