JOB SUMMARY:Within the context of a patient/client and family-centred framework and continuum-based care model and in accordance with established standards of professional practice and the vision, values and policies of the organization, the Occupational Therapist, Spinal Cord Injury (SCI) Chronic Disease Navigation is responsible for delivering client-centered care to individuals recovering spinal cord injury. Working collaboratively with physicians, interdisciplinary teams, and community care providers, the Occupational Therapist ensures comprehensive assessment, planning, coordination, and evaluation of services to address each client’s evolving medical, rehabilitation and community resource needs. Key responsibilities include identifying occupational performance issues, facilitating problem-solving around day-to-day functional challenges, and connecting clients with appropriate community resources to promote independence and quality of life. Provides consultation to internal and external healthcare professionals, supporting assessment and treatment planning, and contributing specialized knowledge in SCI management, including seating, mobility, spasticity, and pressure injury management and prevention. The role also involves continuous evaluation of client progress, adapting care plans as needed, and supporting transitions between acute care and community environments. Contributes to the development and implementation of evidence-based practices, participates in research and quality improvement initiatives, and provides mentorship and support to peers and team members. Advocates and provides education, consultation, and support to client/family and clinicians to ensure best possible outcomes. QUALIFICATIONS:Education & Experience Current full registration as an Occupational Therapist with the College of Health and Care Professionals of BC (CHCPBC). Three years’ recent experience in a rehabilitation health care setting involving spinal cord injury including experience in complex disease management, complex seating and positioning, skin and wound management and spasticity management or an equivalent combination of education, training and experience.Knowledge, Skills & AbilitiesBroad knowledge of complex and aging adult populations living with Spinal Cord Injury, presentations, indicators, needs, complexities, co-morbidities and contra-indicators Broad knowledge of evidence-based clinical practice related to wheelchair prescription, modifications and technologies, seating, positioning, pressure prevention assessment and interventions and access technology.Broad knowledge of evidence-based clinical practice related to upper extremity therapy.Broad knowledge of occupational therapy theory and practice within a client and family centred model of care.General knowledge of Trauma Informed Practice and Harm Reduction approaches and Strengths-based care.Demonstrated ability to promote Indigenous Cultural Safety. Demonstrated ability to provide competent and culturally safe care in a variety of settings and with diverse populations.Demonstrated ability to teach and empower clients to assume control over their health.Broad knowledge of other health care disciplines and their role in client care.Demonstrated ability to communicate effectively in a culturally safe and respectful manner, with co-workers, physicians, other health care staff and patients/clients and their circle of care.Demonstrated ability to function as a team member.Demonstrated ability to provide effective consultation and teaching.Broad knowledge of research process and methodology.Demonstrated ability to work independently and in collaboration with others.Demonstrated ability to establish workload priorities in collaboration with others in a complex health care environment.Demonstrated ability to recognize when referral to another health care professional is required.Physical ability to perform the duties of the position.Skilled in the use of equipment, and in the techniques appropriate for intervention.Computer literacy with work processing, spreadsheet and database skills.The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.
