Through our partnerships, we identify members who are often disconnected from services and work with them to regain stability and re-engage with care.That’s where you come in.
Your Role: Coach, Advocate, Change-Maker
As an Integrated Health Coach, you’ll work hands-on in the field to build trust and move lives forward.
You’ll meet with members one-on-one—at their homes, in the community, or wherever they feel safe. You’ll develop consistent relationships grounded in empathy, and help members define and achieve goals like:
- Getting connected to housing
- Attending a doctor’s appointment for the first time in years
- Navigating substance use treatment
- Rebuilding daily structure, purpose, and hope
You’ll work closely with your team—and use Vanna Connect, our custom tech platform—to track progress, collaborate, and stay organized.

What We’re Looking For
Need to Have
- Over two years of experience working directly with individuals diagnosed with a serious mental illness (SMI), ideally in community-based or field roles
- Experience working in and around the communities of Allen County, Indiana
- Valid driver’s license and comfort traveling in the community daily
- High school diploma or equivalent
Nice to Have
- Spanish fluency (or other languages)
- Certified Peer Specialist training
- Experience supporting individuals with substance use disorders
