POSITION SUMMARY
The Community Living Specialist is in constant face-to-face contact with persons diagnosed as chronically mentally ill. Services and work assignments are primarily in a Facility based setting but could provide coverage for other staff and programs requiring travel to their homes in the community. The Community Living Specialist’s goal is to increase the persons’ capacity for independent community living, through medication management, support, rapport, role-modeling, teaching, and the provision of activities and learning experiences. The Community Living Specialist consistently exercises discretion and sound judgment in the performance of the duties of the position.
Some cases you will be ask and not limited in duties of a case worker
- Serve as the Youth and Family advocate for coordinating all necessary services and will document the youth's referral to and satisfactory completion of each service related to specific court order conditions.
- provide services and supports necessary to assist persons served and their family in achieving and maintaining all goals and court orders.
- serves a broad range case load consistent with credentials and experience
- Consistently exercises discretion and sound judgment in the performance of the duties of the position
MINIMUM QUALIFICATIONS:
EDUCATION & EXPERIENCE:
High School Diploma or GED; plus one (1) adult mental health experience required. Preferred - Associate Degree (Certificated Nursing Assistance (CNA), Medical Technicians (Med Tech.) /or related certified professional. College degree may replace experience requirement.
