JOB SUMMARY: With minimal supervisory oversight, the Health Coach will be responsible for scheduling and conducting group-based, activity-focused coaching sessions for older adults (aged 60+). Each group will consist of 4 to 6 older adults and will meet for four consecutive sessions. Older adults who do not improve their activity levels after group coaching may be offered up to two individual sessions provided by the health coach. All coaching sessions will be offered during standard business hours (8am – 6pm). This position will additionally be expected to support recruitment, data collection, program delivery, and other needs for this and other research studies in the department.
EDUCATION/EXPERIENCE: Bachelor's degree in Nursing, Human Nutrition, Dietetics, Foods and Nutrition, Psychology, or related field of study is preferred. Two years? related health education experience is preferred. LICENSURE, CERTIFICATION, and/or REGISTRATION: Certification in appropriate service area is preferred. ESSENTIAL FUNCTIONS: 1. Provides educational programs and consultation to patients, families, agencies, and community organizations. 2. Interviews patients and families to assess and evaluate health needs and recommends suitable plan. 3. Develops training concepts by attending and participating in medical team evaluations. 4. Provides patient education materials when appropriate (booklets, audio-visual materials for patient/family instruction, handouts). 5. Develops and provides specific planned programs of patient/family education (individual/group) to include training sessions, assessment and evaluation tools. 6. Establishes planned follow-up programs for patients/families needing motivation, reinforcement, and home management techniques. 7. Promotes continuity of patient/family education as necessary with multidisciplinary approach (physicians, nurses, dieticians, drug teams, etc.). Provides referrals to community agencies and resources. 8. Serves as a resource, participates, and teaches in-service programs as needed. 9. Performs other related duties incidental to the work described herein.
