Mental Health Cooperative, Inc. (MHC) was formed in 1993 to serve individuals with severe and persistent mental illness.  Since then, we have expanded our services to children and adolescents with severe emotional disorders across Middle and East Tennessee.

Our sole purpose is to support and treat those challenged with serious mental illness and poverty. Although based out of Nashville, we serve several communities across middle and East Tennessee with satellite offices in Antioch, Gallatin, Dickson, Columbia, Cleveland, Murfreesboro, Clarksville, Cookeville, Chattanooga, and Memphis.

The ICC Supervisor provides support and supervision to ICC team members, including Care Coordinator (CC) and Family Support Specialist (FSS). The Supervisor assists the CC and FSS in serving families with high-risk children/youth/young adults. The ICC Supervisor provides direct support for each ICC team and associates to ensure that all services and support are executed effectively.

Job Title: Supervisor, Intensive Care Coordination

Schedule: Monday – Friday 8:00am – 4:30pm

Salary: $54,000 (base pay) with additional incentive for language skills and behavioral health experience. Mileage reimbursement available.  

JOB SUMMARY: The ICC Supervisor provides support and supervision to ICC team members, including Care Coordinator (CC) and Family Support Specialist (FSS). The Supervisor assists the CC and FSS in serving families with high-risk children/youth/young adults.  The ICC Supervisor provides direct support for each ICC team and associates to ensure that all services and support are executed effectively.


RESPONSIBILITIES:

  • Provide clinical and administrative guidance, training, and support to team that provides Intensive Care Coordination Services.
  • Assist with the referral and transfer of consumers to intensive service programs, external resources, and back to lower levels of care.
  • Promote awareness and education of ICC services within departments at MHC, as well as in the community.
  • Monitor census of ICC programs and promote growth of ICC services.
  • Assist with trainings and continuing education
  • Provide face-to-face weekly supervision to CC utilizing process methods and coaching.
  • Provide weekly supervision / consultation to FSS providing support and coaching around the lived experience of support to families.
  • Facilitate monthly team meetings with all ICC associates.
  • Observe CPTM’s (Care Planning Team Meeting) and complete supervisory field audit monthly to provide CC with feedback, coaching, and support.
  • Ensure CC completes all onboarding assessments with the family and youth from enrollment to discharge as needed.
  • Ensure CC and FSS provide face-to-face direct care to families including support, referrals, and assistance with the development of positive parenting, crisis planning, basic life skills and parenting a child/youth with special needs.
  • Ensure CC and FSS assist families in developing a list of individuals they want to attend the CPTMs (formal and informal supports).
  • Ensure CC provides liaison functions for the team members and service providers.
  • Ensure CC is facilitating CPTMs every 30 days.
  • Ensure CC is coordinating care and communicating with families and support team members regularly (weekly) outside of CPTM’s to check in on progress made towards goals, assisting with barriers, and offering support / resources.
  • Ensure CC and FSS are linking families to community resources, clinical services, and natural supports related to the families’ needs.
  • Ensure CC and FSS are available for crisis calls and collaborating with MCRT and other supports during crises.
  • Ensure that CPTM’s occur within 24 business hours of a crisis.
  • Submit Internal QI Report to Grants Manager monthly.
  • Submit monthly tracking sheets for each associate to Grants Manager.
  • Ensure all prior authorizations are completed for enrollment to ICC.
  • Ensure all ongoing authorizations are completed to continue ICC.
  • Provide team maintenance and transition coordination.
  • Ensure CC and FSS are trained and updated on agency mission, goals, and policies and procedures.
  • Other duties and tasks as assigned.
  • All duties associated with Care Manager Supervisor.

 

SKILLS AND EXPERIENCE:

  • M.A. or M.S. degree in social work, psychology, or related field with a minimum of two (2) years clinical intervention experience in serving youth with SED or emerging adults with mental health conditions. All unlicensed ICC Supervisors must be supervised at minimum by an independently licensed mental health practitioner (e.g., LCSW, LPC, LMFT). Education can be substituted for experience.
  • Ability to create effective relationships with individuals of different cultural beliefs and lifestyles.

  • Strong interpersonal skills and the ability to work effectively with a wide range of constituencies in a diverse community.
  • Ability to develop and deliver case presentations.
  • Ability to analyze complex information, and to define and solve problems.
  • Ability to work effectively in a team environment.
  • Two years of experience of clinical supervision and/or administrative service.
  • Knowledge of the focus population.  Experience working with children/youth and families with significant behavioral health needs.  Ability to lead and provide model for strengths-based, client centered approach to service delivery.
