Full policy is provided upon request<br /> <br /> <b>The impact you will make:</b> <ul> <li>Assist patient with personal hygiene, elimination, / nutritional support, ambulation and exercise according to the patient’s needs and as specified on the CHHA Assignment.</li> <li>Ensure quality and safe delivery of CHHA services per written plan of care.</li> <li>Communicate with RN, Rehab Case Manager or Clinical Supervisor on a regular basis regarding changes in the patient’s condition and/or needs.</li> </ul> <br /> <b>Who you are: </b> <ul> <li>Completion of an approved training program pursuant to Federal/State law.</li> <li>Current, active California CHHA certificate with criminal background check.</li> <li>Current CPR, health clearance and completed skills assessment.</li> </ul> <b>Full job description available upon request</b>
