The Role
MQ Health is currently seeking a, experienced Primary Care Nurse/Care Navigator to provide support and coordination of care for patients accessing multi-disciplinary allied health services through MQ Health. In this role, you will support patients through their end – to – end journey at MQ Health. This includes essential tasks such as actively triaging referrals, educating patients on their care plans and goals, maintaining up to date records of patient interactions and progression, whilst also ensuring necessary appointments and services are arranged for them.
This role also largely supports General Practitioners (GPs) and Allied Health Professionals in the Ryde and Hornsby areas, and will involve travel between MQ Health and practice locations. You will act as the central communication point for GPs, Allied Health Professionals and patients. You will support GPs in the development and creation of care plans and facilitating essential recall and reminder processes.

To Apply
To be considered for this position, you will demonstrate the below requirements in your CV and a 1-2 page cover letter.
Minimum 3 years’ experience in General Practice Nursing or Community Nursing
Experience working in a Care Navigator position or similar
Experience working with a range of allied health practitioners in the provision of patient care across clinic and community settings
Strong communication skills
Strong leadership skills and ability to take the initiative in resolving issues/problems
Demonstrated capability to critically think and respond to clinical changes
Demonstrated ability to multitask in a complex environment
Demonstrated ability in assessing, planning and implementing preventative care approaches to health care
Demonstrated ability in creating and managing patient care plans as part of a multidisciplinary team
Well-developed computer skills including the use of electronic medical records systems, digital health systems, MS Word, Excel and web-based research
