As the Coordinator of Discharge Planning for Behavioral Health, you’ll play a critical role in ensuring patients transition safely and successfully into the next phase of care. You’ll work closely with patients, families, physicians, clinical staff, managed care organizations, and community providers to develop and implement individualized aftercare plans that support long-term wellness.

This role is ideal for someone who understands the complexities of behavioral health and is passionate about connecting patients to the resources and support systems they need. Through compassionate communication and strategic coordination, you’ll help bridge the gap between inpatient care and community-based recovery.

Key Responsibilities

• Collaborate with patients, families, and healthcare teams to identify discharge planning needs

• Coordinate with community providers, social services, and managed care companies to implement aftercare plans

• Verify insurance benefits and entitlements to ensure appropriate post-discharge support

• Facilitate smooth transitions through the continuum of care, including outpatient services, housing, and follow-up appointments

• Maintain accurate documentation and communicate effectively across disciplines

• Serve as a resource for patients navigating behavioral health systems and community services

Minimum Qualifications

Education

• Associate’s degree in a human services-related field or equivalent years of experience required

Experience

• Minimum of 1 year working with behavioral health clients required

• 1–3 years of experience in discharge planning, case management, or behavioral health coordination preferred

Licensure/Certification

• Not applicable

Knowledge, Skills, and Abilities

• Strong communication skills with individuals from diverse socio-economic and educational backgrounds

• Knowledge of community organizations, behavioral health systems, and social service resources

• Ability to navigate complex care environments and advocate for patient needs

• Organizational skills to manage multiple discharge plans and timelines

• Commitment to patient-centered care and continuity of behavioral health support
