MANAGER REHABILITATION SERVICES - Providence Sonoma Home Health

Schedule: Full time Day shift

Manages and coordinates the day to day operations of the home health interdisciplinary rehab team to ensure  quality and appropriateness of care, the availability of services, with appropriate staffing,  facilitates and ensures quality initiatives and program implementation. Manages, maintains and sustains outcomes. These services will be provided in a cost-effective manner and in accordance with the policies and procedures of the organization and conforms to all appropriate accrediting and regulatory agencies.

Required Qualifications: 

• Bachelor's Degree - Related field based on graduating year    
• Coursework/Training - Complete necessary education to attain Physical Therapy, Occupational Therapy or Speech Therapy Licensure in state of California.    
• California Occupational Therapist License upon hire or,
• California Physical Therapist License upon hire or, 
• California Speech Language Pathologist License upon hire. 
• National Provider BLS - American Heart Association within 30 days of hire. 
• 2 years - Physical Therapy, Occupational or Speech Therapy clinical experience.
• 2 years - Management experience in home health or related health field.

Preferred Qualifications: 

• Master's Degree - Physical Therapy, Occupational Therapy or Speech Therapy or, 
• Ph.D. - Physical Therapy, Occupational Therapy or Speech Therapy.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
