The Director of Hospice Intake at Transitions Care will oversees the hospice intake process to ensure timely and compliant onboarding of all new hospice referrals. This position is responsible for building and leading a high-performing intake team, optimizing workflows, partnering cross- functionally with marketing, clinical, compliance and revenue cycle management (RCM) ensuring operational readiness for census growth across all markets.
Major Responsibilities
Team Leadership & Oversight
- Monitor productivity, referral volume, and service levels across both teams
- Provide ongoing coaching, performance feedback, and career development
- Monitor workload distribution, productivity metrics, and quality assurance
- Set and enforce service level agreement (SLAs) for referral response times and admission scheduling
Process Optimization- Intake
- Design, implement, and refine standardized intake workflows
- Ensure seamless coordination with sales/marketing and admissions teams
- Leverage EMR, CRM, and intake systems to reduce handoff delays and documentation errors
- Ensure complete and accurate intake documentation including eligibility and payor information
Compliance & Documentation
- Ensure accurate collection of consents, eligibility, patient documentation and payor information
- Partner with compliance and billing to avoid downstream authorization/eligibility issues
- Stay current with state/federal hospice regulations and adapt processes accordingly
Cross-Department Collaboration
- Act as intake liaison to finance (RCM), clinical services, and sales
- Identify and eliminate intake-related bottlenecks in census growth
- Serve as a key voice in census reporting, referral source analytics, and admission forecasting
Reporting & Metrics
- Track and report on daily/weekly referral volume, conversion rates, time to admission
- Lead monthly operational reviews to assess intake performance across regions and proactively address concerns
- Proactively address referral declines, missed opportunities, and staff performance issues

Education and Experience
- Minimum of three (3) years of leadership experience
- Minimum of Five (5) years of hospice intake experience
- Experience with HCHB (Homecare Homebase) strongly preferred
- Strong working knowledge of the Medicare hospice benefit including reimbursement models
- Experience with/ability to develop and implement metrics/KPIs to track conversion rates, response time, and patient satisfaction
- Comfortable with dashboards, reporting tools (Power BI, Tableau, or Excel).
- Familiarity with referral management platforms
- Strong written and verbal communication skills
- Excellent time management and organizational skills are essential
- Critical thinking skills and ability to solve problems\
- Strong working knowledge of hospice regulations
