Under general supervision, serves as a leader, change agent and coordinator of patient care services; takes a lead role in overall design and implementation of a patient centered care delivery system that facilitates achievement of patient outcomes, supports continuity of care and enhances patient and care giver satisfaction. Is accountable to patients and families for the processes and outcomes of care during an episode of care. Is accountable to peers for the advancement of group practice and to the supervisor for the totality of work performance.
ESSENTIAL FUNCTIONS
- Serves as a role model and clinical resource for expert care and outcomes management for the patient population.
- Assists staff in developing and improving skills in managing therapeutic relationships with patients and families. Assesses staff performance in documentation and provides feedback; monitors improvement plans; develops and implements systems to improve documentation.
- Assesses and improves preoperative patient instructions. Responsible for patient and family education; monitors process and outcomes of patient education; implements improvements as appropriate.
- Develops strategies to promote continuity, consistency and confidentiality of patient information across organizational boundaries; develops systems to streamline and improve patient information management processes.
- Coordinates weekly multidisciplinary clinic and conference; participates as an active and assertive member of the team; assesses and develops staff expertise in functioning in a collaborative role within this team; takes the lead in assuring continuity and consistency of care; incorporates feedback from team members into improvements in unit operations which support patient care.
EDUCATION and/or EXPERIENCE
- Graduation from an accredited school of nursing is required; bachelor’s degree (BSN preferred) or enrollment in Bachelor of Science in nursing program is required. Current licensure in the State of Maryland as a Registered Nurse is required.
- A minimum of three (3) years of relevant clinical work experience is required.
- Ability to respond, investigate, identify and problem-solve regarding issues related to administrative and clinical matters that require immediate and decisive action
- Ability to use computer systems in order to manage patient information
- Ability to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's requirements relative to his or her age-specific needs, and to provide the care needed as described in the department policies and procedures
- Highly effective interpersonal, written and verbal communication skills
- CPR certification is required

The referenced base salary range represents the low and high end of University of Maryland’s Faculty Physician’s Inc. salary range for this position. Some candidates will not be eligible for the upper end of the salary range. Exact salary will ultimately depend on multiple factors, which may include the successful candidate's geographic location, skills, work experience, market conditions, internal equity, responsibility factor and span of control, education/training and other qualifications.
