The Behavioral Health Navigator position addresses behavioral health needs through improving access to behavioral healthcare, educating and empowering patients and their families with high health needs, and improving behavioral healthcare through innovative methods of care delivery and care transition. The Behavioral Health Navigator provides professional case management services for patients by arranging assessments, connecting patients to behavioral health and community resources, developing case coordination plans, collaborating with external services to schedule services, assisting in crisis intervention, and serving as a liaison between Providence Cedars-Sinai Tarzana Medical Center and community resources.

Required Qualifications:

  • H.S. Diploma or GED
  • Upon hire: National Provider BLS - American Heart Association
  • California Fire and Life Safety Card - National Organization within 30 days of hire
  • 2 years experience in behavioral health, care coordination, and/or patient navigation, working with diverse vulnerable populations, managing sensitive information, and proficiency with EMR systems and Microsoft Office 365 Suite

Preferred Qualifications:

  • Associate's Degree in Social work, psychology, public health, community outreach, or related field
  • Bachelor's Degree in Social work, psychology, public health, community outreach, or related field

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
