Salary: starting pay $22.00/ hour (Based on Experience)| Sign-on Bonus $3,000

Position Type: Full time Non-Exempt, 40 hours/ Week

Shifts available: Monday - Friday 8:00am -4:30pm

As the Health Unit Coordinator, you will play a vital administrative and customer service role within our Health Center. You'll serve as the first point of contact for residents, families, and healthcare professionals—ensuring seamless communication and smooth coordination of services. You will be responsible for maintaining complete, accurate and confidential patient medical charts as legal documents, ordering supplies/equipment as directed and answering general inquiries from patients, visitors and telephone callers. You’ll work with the Nursing team and report to the Director of Social Services to ensure every resident has the opportunity to live with purpose and joy.

 

 

What you will do:

  • Provided orientation, guidance, and day-to-day support to department staff.
  • Assist Social Services with collecting Social Histories, PHQ-9s, and Medicare documentation. Supports referrals to psychiatric, home health, and DME services.
  • Provide applicable notices, including but not limited to, discharge and appeal rights to facility residents and/or the residents’ representatives, and follow up on residents’ appeals as necessary.
  • Answers unit calls, greets and assists visitors, distributes mail, and ensures care cards are available to CNAs.
  • Coordinates external appointments and tracks resident whereabouts.
  • Maintains, organizes, and files resident charts, care cards and medical records.
    • Files charts in proper order and keeps track of where charts are at all times. Checks for neatness, torn pages and correct order. Files dictated progress notes, diagnostic testing reports, consults and MAR’s into each resident’s record in accordance with established chart order. Initials reports when filing into chart. Purges records in accordance with policies and procedures. Completes charts of discharged patients and deaths and arranges them in proper order before sending to Medical Records.
  • Performs clerical functions necessary in admitting a new resident and discharging a resident
  • Provides backup for admissions and transcribes physician orders, notifying nurses as needed.
  • Supports MDS Coordinator with weekly insurance documentation.
  • Arranges schedule for residents for external visits; to clinics, x-ray, laboratories, surgery, etc. 
  • Maintains a clean and organized office space.

 

What you will bring:

  • High School Diploma or equivalent. 
  • Healthcare experience required; Health Unit Coordinator experience is strongly preferred.
  • Medical terminology and Electronic Medical Record software knowledge is required.
  • Strong administrative and multitasking skills with high attention to detail.
  • Polished, professional communicator—both in person and via phone/email.
  • Ability to work effectively with ill patients.
  • Working knowledge of Windows office systems and all office equipment preferred.
