POSITION SUMMARY

Reporting to the Senior Nursing Manager, the Nurse Coordinator in partnership with the Senior Nursing Manager, is responsible for training members of the multidisciplinary team to meet the health needs of patients. The Senior Nursing Manager is responsible for leading the activities and daily operations of all health services activities of the assigned Phoenix House program and the Nursing Coordinator will train to the protocols and processes that need to be followed. Areas to ensure training is completed and monitored are health assessment, communicable disease control, state-mandated screenings, first aid, disaster preparedness, immunization follow-up, utilization of approved community resources, and health education of students (if applicable) and staff. The Nurse Coordinator assists to covering nursing shifts, provides training, problem solves the payor source to track onset of coverage for each client, streamlines the scheduling, tracking & monitoring of clients for data collection, outcomes, and follows policies and procedures to ensure compliance with the medical standards set forth by Phoenix House and the Federal Government, State of California, the governing County, and other contract partners.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Arranges and assists in the provision of on-site medical services conducted by the consulting physician/Nurse Practitioner.
  • Arranges off-site medical services as Ensures clients who transition to other levels of care has coordinated medical services and link to medication treatment as needed.
  • Maintains knowledge of and trains to proper practices related to Nursing fundamentals.
  • Is knowledgeable about Communicable diseases including HIV, Pharmacology, and Nutrition and can provide education as appropriate.
  • Assures the accurate and timely preparation and maintenance of all medical records and monitors the quality management of medical personnel in partnership with the Senior Nurse Manager.
  • Responsible in partnership with the Sr. Nursing Manager to audit the self-administration of medication of medical services per policy.
  • Provides client/patient care in accordance with program and individual Problem Lists, which includes:
    • Monitors and reports the behaviors of clients/patients.
    • Ensures the safety of program participants.
    • Ensures incident reporting is completed timely and sent to Quality Assurance.
  • Participates, assists in creating opportunities for and teaches general life skills in the areas of health, fitness, smoking cessation, and nutrition.
  • Responsible for training, assuring, and maintaining confidentiality over all medical records.
  • Responsible for ordering and maintaining medical supplies and all facility first aid equipment and supplies.
  • In partnership with Sr. Nursing Manager is responsible for maintaining medical office and for ensuring all medicines are safely and securely stored. Works in partnership with the Senior Nurse Manger to audit the medication room and make needed corrections at minimum monthly. Coordinates with Sr. Nursing Manager and contracted pharmacy (Genoa) the medication reviews and completes any plan of corrections.
  • In partnership with Sr. Nursing Manger, arranges for additional medical services or health seminars both on and off-site, as needed. Arranges for and leads health groups for clients
  • Maintains all required reporting functions and assure 100% completion of all paperwork to assist the Nursing Manager with required tracking and logging and completion for more accurate outcome measures.
  • Monitors and audits at least once a week that all withdrawal management checks and documentation is completed timely and uploaded into the EHR.
  • Follows and adheres to compliance with local, state, and federal confidentiality laws and regulations.
  • In partnership with the Sr. Nursing Manager, monitors compliance of Incidental Medical Services in facilities certified for these types of services. Reviews IMS requirements monthly.
  • Attends all required staff training sessions in accordance with Phoenix House and state licensing requirements and completes all training on time.
  • Attends and participates in program case conferences and all staff meetings.
  • Adheres to and promotes the Phoenix House Mission, Vision, and Values while acting as a role model for others.
  • Other duties as assigned.

EDUCATION/EXPERIENCE/CREDENTIALS

  • High school diploma or its equivalent required.
  • Licensed Vocational Nurse or higher.
  • Interest in a related field of Substance Abuse or Mental Health preferred.
  • Qualifying experience may be substituted for the required education on a year-for-year basis.
  • Must maintain a valid credential (if required for your position) during your employment.
  • Must obtain TB test clearance and update on a yearly basis.

  • Skills and ability to engage and develop a rapport with patients and families of various backgrounds.
  • Bilingual in Spanish preferred, but not required.
