Care Coordinator

Greater Family Health is a multi-site federally qualified health center providing comprehensive medical, dental and behavioral health services within the Chicagoland area. Your Goal:

The primary purpose of this position is to facilitate coordination of care and continuity of care for all Greater Family Health third-party managed care patients. 

How You'll Help Patients:

• Facilitates referrals for specialty care and testing for applicable third-party managed care patients.
• Completes referral follow ups for specialty care and testing.
• Assists patients in obtaining prior authorization for diagnostic testing, equipment or other services.
• Facilitates coordination of care to individual patient’s clinical measures and care plan.
• Performs various Health Risk and SDOH Assessments and screenings for applicable third-party managed care patients.
• Performs follow-up activities with patients as needed after ER visit and ensures timely follow up with provider.
• Assists patients with adherence to existing self-management goals and/or development of new goals in collaboration with clinical staff. 
• Ensures compliance with Greater Family Health quality assurance and risk management standards.
• Reports to all scheduled work sites and shifts on time and fully prepared to engage in all job responsibilities; location of work sites and shifts will change at the discretion of the supervisor.
• Engage in all offered training, consultations and supervisions made available. Fully and professionally respond to and implement directives and advice offered by all supervisory and training personnel, as well as professionally offered feedback from coworkers.
• Perform other duties as assigned.

What You'll Need:

• High School Diploma or GED Certificate is required. 
• Associate or bachelor’s degree in health or social services field or 3 years of equivalent work experience in health care preferred.
• Medical Assistant, Certified Nursing Assistant, or Community Health Worker experience preferred.
• Bilingual in Spanish is preferred but not required.
