POSITION SUMMARY
The Medical Case Management Grant Counselor, provides support, counseling and referral information for patients and families, and performs complete psychosocial assessments as well as mental health and substance abuse screenings.
The Counselor demonstrates understanding of community resources and communicates and collaborates effectively with agencies and hospital personnel. The Counselor is also responsible for writing medical record notes in a timely fashion, and building constructive relationships with community service professionals. The Counselor participates in the HIV Planning Group and Ryan White funded quality improvement committees.
This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to team work and cooperation.
QUALIFICATIONS
EDUCATION: Bachelor’s degree Social Science required. Knowledge of HIV/AIDS and related issues required for counselor participating in the case management component of Ryan White funded quality improvement committees.
LICENSE/CERTIFICATION:
EXPERIENCE: Minimum of 2 years experience in counseling required. Excellent written and verbal communications skills required. Bilingual preferred. Demonstrates ability to work with diverse populations. Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be required for this position within 30-60 days from date of hire.

WORK ENVIRONMENT
Potential for exposure to the hazards and risk of the ambulatory and hospital environment, including exposure to infectious disease, hazardous substances, and potential injury.

This position reports to department leadership.
