Job Description
Provide oversite of coordination and supervision of the daily implementation of the Individual Specific Plan. Assist in the creation of teaching and support strategies as part of the team process to meet outcomes and action steps and ensure all required documentation is completed within the allotted timeline. To be an advocate for the individual and ensure all health and safety needs are met with dignity and respect. Provide ongoing mentorship, guidance, and training to employees. Maintain positive relations with IDT members, State Representatives, internal departments, and other external entities. Embrace and teach HeartWell Services Mission, Values and Vision.
Position Duties and Responsibilities
1. Provide oversight to coordinators collaborating with Interdisciplinary team members for all individuals receiving Customized Community Supports. Provide oversight to Coordinators working in conjunction with the individual and their team to identify current and future goals, development, and implementation of Teaching and Support Strategies (TSS), outcomes and action steps and all required ISP documentation.
2. Work as an agency liaison with disgruntled interdisciplinary team members and state representatives to ensure satisfaction with services.
3. Independently review written and verbal reports/documentation related to daily progress and provide intervention and training to employees.
4. Provide community inclusion support and behavior management. Independently mentor and train employees with daily functionality of this support process.
5. Advocate and teach others to advocate for the individual(s) supported.
6. Be a positive role model to individuals, employees, and contractors.
7. Provide oversites for all CCS service implementations: including medication management, management of individual funds, ISP implementation / Meaningful Day, ensuring individuals are participating in choosing activities of interest and staying within their financial means, providing opportunities to volunteer and build skills for future employment, transportation needs, community involvement/access, therapies, nursing and nutritional needs.
8. Provide oversite for all employee-based needs and functions including staffing ratios, scheduling, training, assessment, evaluations, infractions, terminations.
9. Evaluate Service Coordinator and employee’s ability to successfully meet the needs of the individual. Provide formal and informal feedback/evaluations and training.
10. Lead the QA/QI committee, including data collection, correlation, and analysis of data.
11. Complete all required training including the DDSD service coordination requirements and work within all HeartWell Policies & Procedures and SOPs.
12. Attend quarterly state provider meetings.
13. Maintain regular communication with the assigned nurse, service coordinator, program director, and HR on all appropriate related matters.
14. Conduct regular audits of all coordinator’s caseloads, documentation, THERAP data, site visits, and online storage.
15. Conduct monthly clinicals with coordination, identify deficiencies and strengths, report to the program director. Provide follow-up to ensure compliance with all Policy and Procedures and State Standards.
16. Active participation in all internal and external audits/surveys.
17. Actively mentor and train employees within the role they currently hold as well as how their role interacts/interchanges with others within the agency. Teach at every opportunity.
18. Work with the Incident & Education Coordinator to support the training of staff, identifying needs for increased training, analyzing GER and ANE data and/ or training improvements for employees.
19. Aid in the maintenance of the IST Tracker by reviewing compliance and removing employees/individuals from databases when no longer with agency.
20. Provide oversite to ensure the attendance of initial meetings and greetings, transition meetings, for coordination, as well as service provisions and staffing requirements.
21. Advise coordination, HR, and Program Director of needed service provisions and staffing requirements.
Listed below are direct tasking responsibilities. Please note this list is not all inclusive; additional tasks are necessary for the success of the service, the individuals and the employees providing the support:
· Backup for On-call service and agency in accordance with current guidelines.
· Ensure management of day-to-day staffing needs.
· Ongoing training and oversite, for service coordination.
· Complete Bi-weekly/monthly documentation verification including billing verification and submission.
· Verify new hire training and competencies.
· Provide training for service coordination.
· Verify service coordination time sheet review on Paylocity and submittal including review of working schedule.
· Communicate daily with service coordinators for updates and observations.
· Aid Coordination with third parties for community activities and volunteer opportunities.
· Verify/complete monthly activity schedules.
· Verify monthly reports/documentation review.
· Ensure team driven monthly emails.
· Verify monthly updates to annual/semiannual ISP reporting.
· Verify that the ISP vision, outcome, action steps, and meaningful day match in THERAP and are up to date. Ensure template in THERAP is used as directed.
· Verify client funds reconciliation (Ledger/ receipts).
· Ensure nutritional recommendations are met through reviewing mealtime needs for individuals and ensuring employees are aware of needs.
· Teach and assist Coordinators with implementing the RORA process.
· Spot check coordinator site visits monthly.
· Documentation audits.
· Attending meetings: ISPs, clinicals, administration, department specific when coordinator is out sick, on PTO or when the position is opened.
Collaborate directly with individuals during times of staffing needs
Requirements
Must be over the age of 25
Completed a minimum of 5 years’ service coordination or case management in same or similar field.
Minimum – High school diploma or equivalent
Current and Valid Driver’s License
Safe insured/ registered vehicle
Pass a background check and drug test.
