- Supervise home health aides with nurse delegation instruction.
- Conduct initial patient and home safety assessments.

- Communicate effectively with referral sources and in a timely manner.
- Complete accurate clinical documentation.
- Currently licensed and in good standing.
- Meets the health requirements for employees providing direct patient care.
- Read, write, speak, and understand English as needed for the job.
- Current CPR Certification.
- Have a valid driver's license and an insured automobile and the ability to travel within our service area which is a 45-minute radius of Des Moines.
- Excellent communication and documentation skills.
