This is a per diem/on call position.

The Hospice Triage Nurse provides holistic care to terminally ill pediatric or adult patients and their families by assessing the needs of the patient, and then planning, coordinating, implementing, and as needed, revising the Plan of Care. This position works collaboratively with the interdisciplinary hospice group (IDG) with the goals of anticipating changes in the patient's condition, avoiding crises, promoting patient and family choice, ensuring continuity of care, and enhancing the quality of life of dying patients. Shift, schedule and caseload of shifts and caseload may vary according position assignment.

Required qualifications:

  • Associate's Degree in Nursing.

  • upon hire: California Registered Nurse License.
  • within 30 days of hire: National Provider BLS - American Heart Association.
  • Three (3) years nursing experience.

Preferred qualifications:

  •  Bachelor's Degree in Nursing.
  •  Prior Hospice, Homecare, Long Term Care, medical/surgical, and/or oncology experience preferred.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
