Job Summary:

The Day Treatment Clinician provides clinical case management as well as individual and family therapy for an evolving caseload of clients. Works collaboratively from intake to discharge with internal and external multi-disciplinary team (MDT) members to ensure continuity and consistency of care for clients and families.  Provides clinical case direction and recommendations to both cottage and classroom milieus as well as the MDT. Actively participates in individual clinical supervision, clinical team meetings, and mirror team consultation.  Plans and conducts weekly therapy groups. Maintains up-to-date documentation and overall quality assurance for assigned clients.

Essential Duties:

  • Conceptualizes the therapeutic needs, strengths and direction of assigned cases. Identify case management needs, utilize CCAR, CANS and other assessment tools to formulate effective treatment planning.
  • Develops and maintains an awareness of the admissions process and criteria for each level of care.
  • Establishes criteria for client discharge from the time of intake.
  • Conducts individual and family assessments and develops quality Service Plans, with particular attention to interventions, level of care and disposition planning.
  • Completes in a timely manner documentation of individual and family sessions, case management contacts, monthly progress reports, group therapy notes and other required forms of paperwork.
  • Facilitates monthly staff meetings with the MDT to ensure treatment plan implementation.  Attends other treatment-related meetings including county Human Services Team Decision-Making (TDM) meetings and/ or court hearings.
  • Prepares for and attends internal psychiatric appointments with clients.
  • Delivers weekly group therapy services in residential and/or day treatment environments.
  • Completes other related duties as assigned.

Education/Experience/Licensure:

  • Master of Social Work or a graduate degree in a related field is required.
  • Post-graduate work experience treating families and children is preferred.
  • Must be registered and in good standing with DORA (Department of Regulatory Agencies)
  • Licensed or License eligible (LCSW, LPC, LMFT)
  • All candidates must have, or qualify to obtain a current Colorado driver license. (Must obtain Colorado license within 30 days of employment.) 
  • Employment Requirement: All employees must be fully vaccinated against COVID-19 by September 30, 2021. All persons offered a position will be required to provide valid proof of vaccination prior to starting employment.
