       Salary Wage Range:$36.01-$46.31

Essential Functions:
 Conduct assessments, screenings, direct case management for Medication Assisted Treatment (MAT) and other outpatient clients with individuals, families, couples, and group counseling and random drug screening.
 Work closely with the MAT provider to coordinate care for MAT clients, educate clients, health care clinics on clients on MAT, and SBIRT treatment services.
Assist patients with learning coping skills and creating stability in their lives.
 Identify and assist with removing barriers in relation to patient mental health stability and overall therapeutic goals.
 Functions as a member of a clinical team to coordinate mental health interventions with other members, providing internal and external consultation and case management of assigned clients as needed.
Directs, coordinates, and maximizes the integration and cooperation of Behavioral Health programs with other county services and local community resources within an assigned region or service area by facilitating collaborations and partnerships between representatives of the county, contract providers, and non-profit agencies.
Coordinates care that is safe, timely, effective, efficient, equitable, and client-centered.
 Maintains the responsiveness of Behavioral Health systems by identifying the needs of the community, evaluating the quality of behavioral health services, addressing issues and concerns, resolving problems and developing plans and objectives.
 Participates in developing and implementing long-range plans pertaining to behavioral health services, such as healthcare integration and evidence-based prevention and treatment approaches.
Interacts with other organizational providers, community partners and stakeholders to share information and resources and coordinate activities in order to ensure that service deliveries meet goals, standards, and expectations.
 Serves as a point of contact for clients and their advocates who are experiencing difficulties with behavioral health services and facilitates solutions to resolve such difficulties.
 Identifies issues that require additional investigation and evaluation, validates discrepancies and ensures appropriate follow up.
 Responds to inquiries from other healthcare organizations, interfaces with internal and external customers on day to day issues as they arise.
 Independently responds to inquiries, concerns, and complaints.
 Ensures that program standards for service delivery are in place and maintained.
 Analyzes and develops methods of improving work measurements or performance standards.
 Communicates with the Behavioral Health Director to identify gaps, needs and priorities, and makes recommendations for compliance and improvement.
 Serves as a resource for employees in the area of Behavioral Health.
Keeps current on Oregon Administrative Rules (OAR) standards and changes and communicates them to the staff.
 Coordinates and manages special projects as assigned.
 Authorized to serve both eligible and non-eligible beneficiaries in an outreach capacity both on and off site.
 Assists with medication prior authorizations.
Provides nursing support to medical department, in times of need.
 Participates in community events to provide education on MAT services.
 Performs a variety of other duties as assigned by supervisor.
Education, Experience, and Licenses:
 Graduate of an accredited school of nursing with current and valid nursing licensure to practice as a Licensed Practical Nurse or Registered Nurse in the state of Oregon.
 Three (3) years of experience working with an Opioid Treatment Program (OTP) or Medication Assisted Treatment (MAT) program preferred.
OR equivalent combination of education, training and experience.
Driving Requirements:
 This position may require driving.

Qualifications, Registrations, and Certifications:
Must have knowledge of EHR software or a similar practice management system.
Knowledge of current Behavioral health theories, practices, and programs.
 Knowledge of principles and practices of evidence-based prevention and treatment approaches
Planning, administration, development, implementation, monitoring and evaluation concepts, methods and techniques, as applied to Behavioral Health Programs.
 Ability to perform a variety of duties, often changing from one task to another of a different nature without loss of efficiency or composure.
 Ability to work with all levels of individuals, including disgruntled people in an effective manner.
 Demonstrated working knowledge of computer software such as Microsoft products and related programs.
 Ability to operate standard office equipment (i.e. copier, fax machine, personal computer).
 Excellent interpersonal skills oral and written.
Must be able to maintain confidentiality in all matters.
 Must possess strong organizational skills
 Requires strong commitment to accurately completing task, solving problems, demonstrating initiative, organization, and attention to detail.
 Ability to speak before an audience effectively, compile written reports, read and interpret various documents, and obtain the cooperation of Tribal programs required.
Knowledge of and sensitivity to Native American culture is required.

Environmental Factors and Conditions/Physical Requirements:
 Continually requires the ability to express or receive detailed information or important instructions through oral communication.
 Continually requires working with fingers rather than the whole hand or arm. Requires repetitive movement of wrists, hands and/or fingers. Often requires walking or moving about to accomplish tasks.
 Requires standing and/or sitting for sustained periods of time, raising objects from a lower to a higher position and/or moving objects horizontally.
 Occasionally requires lifting of up to 40 pounds.
 Due to the nature of the interactions with other employees and the public we serve, employees may be required to provide documentation of, or receive, certain health vaccinations.
This position is frequently subject to inside environmental conditions which provide protection from weather conditions, but not necessarily from temperature changes, and is occasionally subject to outside environmental conditions.
 May be called upon to serve as a Disaster Reservist during emergencies.
 Local travel is occasionally required. Travel outside of the state is infrequently required.
