We are currently hiring for two Case Managers for our Roseburg location.

SUMMARY OF DUTIES
The purpose of this position is to assess initial applicants, their family situation, and on-going participants to develop, coordinate, and arrange a variety of services to allow elderly and disabled adults or medical patients about to be discharged from a hospital to get the continuity of care needed or to allow families in Oregon's family assistance programs to become self-sufficient while helping them to meet their basic needs.
As a Case Manager, you will:
Screen clients for medical, social, and financial eligibility and assesses clients' medical, physical, and mental condition and level of function. Interview client, physicians, hospital discharge staff, family members, and others.
Match clients with providers by determining appropriate service placement and payment level. Monitor on-going needs and provider services through regular contact with providers and clients including staffing client needs with providers, giving on-going training to providers, and directing providers in client care. Maintain appropriate records.
Utilize judgement in recognizing and assessing client’s health and functional status and be knowledgeable of issues representing potential abuse and neglect.

Minimum Qualifications:
A bachelor's degree in a Behavioral Science, Social Science, or a closely related field.
OR
A bachelor's degree in any field AND ONE YEAR of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).
OR
An associate degree in a Behavioral Science, Social Science or a closely related field AND TWO YEARS of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).
OR
Three years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).

We are looking for candidates with:
Experience assessing the eligibility of clients, documenting case information accurately, and identifying potential risks or changes in health and well-being.
Experience collaboratively with healthcare providers, social service agencies, and families to develop and implement effective care plans.
Ability to provide compassionate, client-centered support by clearly explaining services, addressing concerns, and advocating for individual needs.
Experience taking initiative in managing caseloads, identifying client needs, and ensuring timely coordination of services with minimal supervision.
Providing sound judgment to assess complex situations, determine appropriate services, and develop tailored care solutions.

Working Conditions:
This position will work out of the office, remote work is not an option at this time. The normal schedule for this position will be Monday – Friday, 8am to 5pm. The worker is required to make home visits to various areas of Douglas County. Case Managers may be exposed to unclean living conditions, communicable diseases, second-hand smoke and pets. The position requires regular contact with clients/family members/other contacts who may be emotionally fragile, demanding, and potentially hostile. The ability to prioritize vulnerable clients, think critically in crisis situations, and communicate appropriately in stressful situation is essential.
There is required use of a laptop computer during home visits; strong computer skills are required to navigate a variety of mainframe screens and search online for pertinent rules and regulations pertaining to eligibility and program specifics. Multiple and restrictive Federal and State regulations apply. Numerous procedures exist for processing assignments that vary in nature and must be completed timely and accurately. The worker must be able to communicate program requirements in a manner that facilitates understanding to the general public and mentally and/or physically impaired individuals. Physical demands of the position in the office are limited.

The Oregon Department of Human Services does not offer visa sponsorship.
