Job Responsibilities:
• Provides for safe and effective use of medications through comprehensive medication therapy management, consulting with members of the healthcare team, and obtaining prior authorizations.
• Communicates with patients, clinicians, and medical providers in a clear and concise manner.
• Prevents, detects, and resolves medication-related problems and makes appropriate, patient-centered medication therapy recommendations.
• Counsels patients and caregivers on medication use and disease management, safe administration measures, and side effect management strategies.
• Provides clinical consultation and clarification to practitioners such as appropriate, cost-effective therapeutic alternatives to clinical and medical staff as needed.
• Teaches, mentors, and precepts pharmacy students and residents
• Performs additional duties as assigned.
• Initial chemotherapy ordering.
◦ Analyzes medication for drug-drug and drug-disease interactions, lab values, and patient functional status.
◦ Enters initial dosing for oral chemotherapy, then sends to the provider for verification.
◦ Sends prescriptions to Marshall pharmacy if applicable.
◦ Completes prior authorization.
◦ Aids in financial assistance if needed.
◦ Follows-up with specialty pharmacy/patient on delivery of medication.
▪ Addresses patient concerns and counsel on dosing instructions.
• Monitors all oral chemotherapy agents.
◦ Analyzes patient labs during visits.
◦ Sees patients on follow-up appointments for symptom management.
◦ Evaluates for toxicities.
• Drug level monitoring when consulted by providers.
• Performs additional duties as assigned.
EDUCATION: Must hold current Pharmacist’s License and have or obtain licensure in the state of West Virginia.
EXPERIENCE: Residency or Board of Pharmacy Specialties (BPS) Certification preferred or must be obtained within 3 years of hire; Minimum 2 years’ experience in clinical patient care management preferred.
