This employee operates the day-to-day lead program for the region.
  • Achieves individual quality and productivity goals in accordance with established objectives
  • Assists with driving revenue growth across the region
JOB FUNCTIONS
  • Follows up on needs for the patient from Chart note review
  • Identifies and processes leads across assigned region
  • Acts as a liaison between hospitals, clinical staff, health plans, providers, and patients to process referrals
  • Documents referrals, communications, actions, and other data
  • Trains center staff on leads, managing best practices
  • Calls MD and Patient to discuss leads and needs
  • Supports the organization in achieving targets
Qualifications Education
  • High School Diploma or General Education Degree (GED) Required
Work Experience
  • Experience working with homecare patients would be Preferred
Knowledge, Skills, and Abilities
  • Read, write, and perform mathematical calculations and the English language effectively
  • Work independently
  • Detail-orientated with excellent organization
  • Proficient in Microsoft Office programs
  • Plan, review, supervise, and inspect the work of others
