The Nurse Navigator will be responsible for the coordination of care of patients in the Perioperative Services department. The Nurse Navigator will collaborate with the Perioperative Services staff, physicians, physician’s offices, patients, and all CHH customers to increase efficiencies and improve the patient experience.  The Nurse Navigator will be an advocate to help patients and families navigate the process of surgery to decrease wait time, improve patient experience, and increase OR efficiency by coordinating times and direction.  The Nurse Navigator will collaborate with physicians and physician’s offices to communicate patient arrival times, order of patients scheduled for procedures and other information as needed.  The navigator directs the flow of patients into Perioperative Services allowing for an exceptional customer service process for our referring physicians and facilities. The Nurse Navigator is responsible for maintaining the surgery schedule by keeping it current and accurate daily. This includes entering all necessary information into Cerner each time, and ensuring each patient has a pre-certification number prior to posting the case. This role plays an integral part of patient satisfaction by recognizing potential conflicts (i.e. room, between areas, equipment, etc.) and taking the necessary steps to ensure the patients’ procedure takes place without cause by timely notification of the appropriate personnel to resolve the issue. Attention to details, effective communication skills, and critical thinking must be utilized at all times on all shifts. Will work closely and collaborate with the Service Line Coordinators/Charge Nurses/Supervisors. Ensures all scheduled cases have current, correct preference cards to avoid waste of materials. Ensures efficient scheduling of daily cases to include appropriate selection of surgeon specific preference card for the scheduled case

  • Serves as an essential link between patients and Perioperative Services
  • Responsible for outreach efforts to establish and maintain positive working relationships with key customers (physicians, office staff, nurses, etc.).
  • Tracking of quality metrics to measure success as well as implement change, as needed.

 

The Nurse Navigator exercises independent judgment and professional discretion in the practice patient flow throughout Perioperative Services.  The Nurse Navigator serves as an advocate to patients and their families. The Nurse Navigator monitors patient’s arrival times and wait times and assists with providing updated information on start times and potential delays, extending into the patients stay in the PACU.  The Nurse Navigator is proactive and uses collaborative problem solving skills to meet the needs of the patients and physician.

 

The Nurse Navigator will serve as a resource person to assist with other assignments as assigned. 

Qualifications
  • Bachelor's degree in Nursing is required
  • AHA BLS is required
  • Current RN licensure in WV is required
  • Minimum of 2 to 5 Years of Position-Related Experience
  • 5+ Years of Position-Related Experience is preferred
